Essential skills are effective coordination and communication to provide continuity of care for designed caseload; proficient in Microsoft Word and maintenance of records, provide education to staff and clients, Must have current LPN licensure in Kansas, experience preferred in DD/ID field, home health, skilled nursing or other community based healthcare.
